A bad repair gets
paid for twice.

The weld is the cheapest line on the invoice. When a repair fails inspection, you don't redo the weld — you redo the scaffold, the blinds, the crew mobilization, and the outage. Here's the arithmetic, and how to stay on the right side of it.

From fifteen years under the hood

Quality is easy to promise. It is harder to hold.

Every outage starts the same way. The contractor talks quality and means it. Then the schedule takes over. The general foreman leans on the foreman, the foreman leans on the crew, and the crew has done this long enough to know which welds get a real inspection and which get a look at the end, if that. Finishing early and handing the plant back feels like a win. Sometimes it is. More often than anyone admits, it is not.

In fifteen years under the hood I watched the same corners get cut on nearly every turnaround. Interpass temperature never checked while the welding ran. Filler metal pulled from the oven and left out in the weather. Fit-up gaps of five-eighths and three-quarters of an inch on joints where the WPS called for a quarter inch, give or take a sixteenth. None of it shows up on a final visual. All of it shows up later.

Storage tanks are where it hides the longest. I have walked tanks carrying hundreds of non-conformance patches, plenty of them in the critical zone, plenty that never met basic repair standards or RAGAGEP. Patches crowded too close together. Patches too small to carry the load. Weld seams lapped over one another. Every one of them avoidable.

That is the work we do. Not reviewing a job after the crew has gone home, but standing on it while it happens, with an eye that already knows where these repairs tend to go wrong. Quality that gets watched is quality that holds.

The oversight line is the smallest number on the table. It's also the one that decides which column you live in. And this table is the cheap version of failure — it assumes the repair gets caught at inspection, not in service.
Field-earned, not theoretical

The five places repairs go wrong

Fifteen years making code repairs, then an inspector's card. These are the failures I keep seeing — and none of them are welding-skill problems.

  1. No qualified procedure behind the weld

    The WPS doesn't cover the base metal, the thickness range, or the position — or there's no PQR behind it at all. The weld can be beautiful and still be scrap, because ASME IX doesn't grade on looks.

  2. Welder quals that don't match the work

    Continuity lapsed, wrong process, wrong position range. It surfaces when the owner's inspector asks for qual records — after the weld is in. Verifying this takes minutes before mobilization.

  3. Preheat and interpass nobody watched

    The torch was there, the crayon wasn't. On thicker sections and harder alloys, skipped preheat is a cracking sentence served weeks later — long after the crew is gone and the scaffold is down.

  4. The wrong filler in the right rod oven

    Material substitutions on the fly, unmarked electrodes, or filler that never matched the base metal chemistry. Five minutes of material verification against the repair plan beats one metallurgical surprise.

  5. Paperwork that can't prove any of it happened

    To the owner's inspector and the jurisdiction, an undocumented repair is no repair at all. Missing NDE records, unsigned travelers, no material certs — the weld holds, the file doesn't, and the file is what gets inspected.

Before an arc is struck

  • Repair plan reviewed against API 510 / 570 / 653 and NBIC
  • WPS/PQR lineup for the actual materials & positions
  • Welder qualification & continuity check
  • Filler metal & base material verification
  • Hold points agreed with owner & contractor

During the work

  • Fit-up and joint prep inspection
  • Preheat & interpass temperature verification
  • In-process visual — root, hot pass, progression
  • NDE coordinated at the right stages, not after burial
  • Problems flagged while they're still cheap

After the last pass

  • Final visual to the governing acceptance criteria
  • Pressure / leak test witness where required
  • Documentation package assembled & signed
  • Records that satisfy the owner's inspector & jurisdiction
  • Straight answers on anything that needs a second look
